This really depends on age, health, other medications and how much it bothers the patient. The most effective treatments are oral medications (there are now three good ones in the U.S. (Lamisil, Sporanox, Diflucan). All have potential side effects that you must discuss with your doctor.
Topical medications are only occasionally effective but have few side effects. Still, many patients opt to try them first or exclusively because of the safety profile. Sometimes removal of the nail (in a way that lets it regrow) and topical treatment will work.
