Doc , I have had endo- surgery 14 months ago . Still have very obstructive symptoms. inferior turbe enlarged middle turbs moderate. even the upper tur. swollen HAD ct. and found marked mucosal diease right frontal ehmoidal , with lesser diease bilateral and contigious musal diease superomeal aspect of right middle meatus The pressue / chronic obstruction is interruping my life what is the next step

More information would be required before making any specific recommendations. Surgery is usually performed to alleviate obstruction to sinus drainage pathways. However, surgery by itself is not always curative for chronic sinus disease. This is particularly so if there are significant environmental factors involved such as allergy. Many patients require ongoing medical therapy even after successful surgical correction of obstructed sinuses. Such medical therapy may include the regular use of topical nasal steroid sprays, mucolytic agents to thin mucous, and the regular performance of sinus irrigations. I recommend the use of a special pulsatile sinus irrigation device known as a Grossman sinus irrigation tip which is attached to a Water-pik. Dilute salt water solutions are then used to flush the sinuses out on a daily basis.
