From time to time, most otolaryngologists who treat large numbers of patients suffering from chronic sinusitis will have an occasional patient who has persistent sinus disease despite aggressive medical and surgical interventions combined with detailed attention to environmental factors such as allergy, irritants in the workplace, cigarette smoke, etc. Assuming surgical relief of anatomical obstruction to the normal sinus drainage channels has been appropriately accomplished; the possible role of allergy or irritants in the environment has been addressed; you have remained faithfully on topical nasal steroids; and you continue to have recurrent infection, then the only other therapy that I could suggest would be the regular daily use of pulsatile nasal irrigation using dilute salt water and a Grossan nasal irrigator tip attached to a Water-pik. Occasionally, I will use this device to administer topical antibiotic solutions into the nasal and sinus cavities as well.
