I had sinus surg. osteo bialt & nasal antral window. 6m, 1 year post op I still have pain @ bridge of nose that gets worse when bending over , lying downward , and really hurts when I caught a virus . Ct showed marked mucosal diease in rt post.ethmoid and lesser thicnening bilat anter& poster & contigous musal diease superomeal aspect of right max

I had my scan looked at for additional opinion , Dr. stated I might be at risk for osteomylitis, saw irregular signal , widing of boney profile, in poster. orital area, I have purelunt drainage , what should I do ?. could you evaluate or have scan evauated . I could send Via mail. Ceftin helps with the pain ? If no infection then what is cause and effect? I had polypoid diease bilateral osteomeal surgery . He did not go futher !

It sounds as though you have persistent chronic sinusitis. If you have had comprehensive medical management including sustained (several weeks) of antibiotic therapy (preferably culture directed), nasal steroids, mucous thinning medications, regular nasal irrigations (I recommend the daily use of pulsatile nasal irrigation using a Grossan nasal irrigator tip attached to a Water-pik with dilute salt water solutions or perhaps antibiotic solutions), allergy evaluation/management, and despite all of this you continue to have purulent sinusitis with CT evidence of mucosal disease, then surgery is most likely indicated. Most likely additional surgery would necessitate a complete ethmoidectomy with sphenoidotomy, antrostomy, and exploration of the nasofrontal region.
