I have been having increasing episodes of fluctuating hearing loss with severe tinnitis. Tests for lupus, rheumatoid factors are negative. No trauma or loud music or diving in past. Am currently on medrol dose pack with no relief. Am positive for herpes 1 &2 titers. What is prognosis or treatment. I am a 37 year old female, who relies of hearing for occupation (registered nurse). Thank you,

Based on the information you have provided there is not quite enough to formulate a diagnosis nor for that matter a prognosis. There are a variety of conditions, both local and systemic, that could account for such symptomatology including endolymphatic hydrops, autoimmune disease, infectious conditions, metabolic conditions, endocrinologic conditions, allergy, etc. A thorough assessment is necessary including detailed history, neuro-otologic exam, comprehensive audiometric studies including objective documentation of fluctuation, radiographic studies (MRI), as well as hematologic, metabolic, and serologic studies to evaluate symptoms such as you have described that are persistent.
